DISTRESSING DISCOVERY: An investigation into a Massachusetts pharmacy at the center of a deadly meningitis outbreak has found conditions including leaky boilers, dirty floor mats and drugs shipped out without sterility tests.

PLAN OF REMEDY: State officials said the preliminary probe cannot prove conclusively what caused the outbreak, but they plan to tighten oversight of similar companies.

HIGH TOLL: The outbreak of fungal meningitis, an inflammation of the lining of the brain and spinal cord, has sickened 308 people in 17 states, including 23 who have died.
