SCOTTSDALE, Ariz., Oct. 19, 2015 (GLOBE NEWSWIRE) -- Matrix Medical Network® (Matrix), the leading provider of in-home assessments and care management, will present a special information session about how health plans can impact high-utilizing members through chronic care management at America’s Health Insurance Plans (AHIP) National Conferences on Medicare and Medicaid on Oct. 21, 2015 in Washington, D.C.
Matrix Vice President of Chronic Care Heidi Wold, MSN, ARNP, ANP-BC, will co-present with Vice President and Chief Medical Officer of Government Programs Paul Alexander, MD, MPH, of Horizon Healthcare Services, Inc., New Jersey’s oldest and largest health insurer. The session, Social Determinants in Vulnerable Populations: Impact on Medical Management, will take place on Oct. 21 at 11:30 a.m.
During the session, Ms. Wold and Dr. Alexander will discuss how social determinants affect members’ health and the importance of continuity of care in mitigating these impacts. High-utilizing members disproportionately suffer from behavioral issues, access to care, financial challenges, mental illness and other hurdles. Matrix’s Care Direct program has been designed to help health plans more effectively manage these difficult situations and address these wide-ranging challenges. Further, Ms. Wold and Dr. Alexander will speak to Horizon’s results with health plan members currently engaged in the program, and share their insights on how to successfully address social determinants facing these members.
“In the field, we are seeing a population that is struggling to meet their basic needs while also facing behavioral and mental health issues,” said Ms. Wold. “Managing their chronic conditions is often too far down their list of needs to garner the attention it requires. These people are overwhelmed with life challenges. The only way to make a difference in this vulnerable population is to treat each member holistically – building a personal relationship with them, addressing their immediate health issues and helping them meet their basic needs.”
Matrix Care Direct is a comprehensive in-home chronic care management solution that helps health plans engage high-utilizing members in the ongoing management of their chronic care needs. Care Direct Nurse Practitioners work collaboratively with the member’s primary care physicians and other providers to identify and close gaps in care, and to deliver positive and sustainable outcomes, even among the highest-risk populations.
