Dart Chart Systems to Provide Proactive Analytics including Re-hospitalization Risk Management Tools for Medicare Payment for NTT DATA Net Solutions Point-of-Care

Dart Chart Systems is currently providing analytics nationwide, and will be rolling out additional features to support skilled nursing facilities needing to prevent re-hospitalization.
