DETROIT, Oct. 9, 2012 /PRNewswire/ -- The Michigan Association of Health Plans (MAHP) presented Meridian Advantage Plan of Michigan (HMO SNP) with a Pinnacle Award for Best Practices during an awards ceremony on Sept. 18 at The Lansing Center in Lansing, Mich.
Meridian Advantage Plan of Michigan's submission, entitled "Integrated Information for Integrated Care," earned the honor in the Care Management for Medicare Populations-Special Needs category. This is the first Pinnacle Award for the Medicare health plan.
