GETTYSBURG, Pa.--(BUSINESS WIRE)-- OPEN MINDS has announced today that it will co-produce with Netsmart Technologies a new executive web briefing titled, “What It Takes to Be a Health Home: Moving Toward an Integrated Care Model.” This information-packed session, scheduled for November 13, 2012 at 2:00 PM EST, will provide executive teams with a roadmap to creating a health home delivery system model. The web briefing is free through underwriting provided by Netsmart.

“Every specialist organization needs a strategy for participating in integrated care models,” said Monica E. Oss, OPEN MINDS Chief Executive Officer. “Health homes are one of the key models emerging. Whether it’s the federally-defined health homes in Medicaid plan — or the emerging specialty primary care medical home models in Medicare and commercial health plans — these are the future models for serving consumers with cognitive disabilities. This briefing is the ‘how to’ crash consultation in developing and managing these innovative care systems. Participants can submit their questions to me in advance of the briefing – and we’ll be prepared to answer them.”

The web briefing will be presented by Larry Seltzer, Netsmart’s Director, Architecture & Solution Strategy, and Monica Oss of OPEN MINDS. The first forty-five minutes of the briefing will focus on health home service requirements and how to meet the expanded tech requirements of integrated care. The last fifteen minutes will feature a question-and-answer session moderated by Monica Oss with questions submitted by participants. The discussion agenda includes:
